DICE Subordinate Certificate Authority Application

In order to apply for a DICE Subordinate Certificate please follow the procedure below:

  1. Complete the form below.
  2. Once the form has been submitted and DCP has verified the information an email will be sent to confirm the signatory information so a DCP, LLC DICE Subordinate Certificate Authority Services Agreement can be sent to the signatory via DocuSign. (Note we will not countersign at this time until all the steps below are complete and deemed accepted by DCP LLC)
  3. Once the signed Agreement is received an online account will be set up for you to be able to upload the necessary supporting documentation. The following documentation is required and will be reviewed and approved in order to become a DICE Subordinate CA holder:
    • Evidence of a minimum of $5M in Errors and Omissions Insurance
    • Articles of Incorporation or Statement of Good Standing (or similar)
    • A written and maintained Certificate Practice Statement (CPS)
    • A FIPS 140-2 Level 2 compliant device for generating and storing CA signing keys
    • Any other information reasonably requested by DCP
  4. Provide Company’s IANA Private Enterprise Number
    • The Licensed Company must include its IANA Private Enterprise Number and IANA registered name in Certificate Signing Requests. As such if a Licensed Company does not have an IANA Private Enterprise Number, it must obtain one by submitting a request to https://pen.iana.org/pen/PenApplication.page
    • NOTE: It may take up to 30 days for the IANA application to be processed.

Evidence of the requirements above must be submitted to DCP for approval. DCP shall process the request within 10 business days.

If the information is accepted DCP LLC will countersign the DCP, LLC DICE Subordinate Certificate Authority Services Agreement and will send a Signing Facility User’s Guide for ordering.
